CPT 99483 is for a comprehensive cognitive assessment and care planning visit, not a single memory test. The clinician reviews cognitive concerns, health history, medications, everyday functioning, safety, and available support. Information from someone who knows the patient well is also an important part of the assessment. The findings help shape a written care plan.
A clinician may recommend a separate assessment when signs of cognitive impairment are noticed during a routine or Medicare Annual Wellness Visit.
